The Wheelchair Ramp Referral Process
IF YOU ARE A CLIENT IN NEED OF A RAMP, DO NOT APPLY. We can only accept referrals from qualified organizations and individuals.
Before you can submit a ramp referral, you need to meet the following qualifications. Otherwise the referral will be denied.
- You must be a social services agency representative, community service representative, doctor, nurse practitioner, other healthcare professional, or an EMS/Fire Rescue/Police liaison?
- You must verify that this ramp referral is the first and only referral made for this client. You have asked the Client if they have asked other agencies or qualified individuals if they have already submitted a referral on their behalf. We can only accept ONE referral per physical address. Any duplicates will cause ALL referrals to be denied and rejected.
- You must complete the referral form COMPLETELY, including and most importantly, your name, your title and your organization’s name, email address, contact telephone number and alternate contact information. If this information is not included on the referral form will be denied.
- The client must meet your income requirements and medical needs for a wheelchair ramp.
- Due to the number of referrals we receive, you must communicate to the client that there may be a significant wait time for a ramp to be constructed, sometimes as long as 3-6 months or longer. If this does not meet the client’s needs, do not submit a Ramp Referral form.

Priority is given to clients with medical needs and disabilities, including dialysis patients, significant mobility issues (wheelchair / walker requirement), those with mobility restrictions who are living alone and are unable to exit their home, and all clients must have a financial need and cannot pay for their own wheelchair ramp.
On the form, there are three priority classes that are shown with a “button” at the bottom of the referral form. If you feel your client needs priority and he/she meets the requirements above, please indicate so (and why) in any of the free-form text boxes. A common example is an acute patient about to discharge from a medical facility to the home – e.g. auto accident amputee living in a mobile home, etc. We focus on the clients with the greatest need and are financially unable to pay for ramps themselves.
